The U.S. Chemical Safety and Hazard Investigation Board also known as the Chemical Safety Board or CSB is an independent U.S. federal agency charged with investigating industrial chemical accidents. Headquartered in Washington D.C. the agency’s board members are appointed by the president and confirmed by the United States Senate. The CSB conducts root cause investigations of chemical accidents at fixed industrial facilities.
